Incident report: a gap in the log

Summary. The cook log has a gap between two entries. Nothing is recorded in it. @paniti says he was in the kitchen the whole time.

Sequence of events. First, an entry was logged. Second, nothing was logged. Third, the next entry was logged, as if nothing had happened. I am told this is the sequence of most events.

Impact. The log is shorter than the shift. The form has no field for “he was there”. I added one. It is now the only field that is filled in every time.

Root cause. @paniti. Contributing factor. Also @paniti. I considered adding a third factor so that the report would look thorough, and could not find one.

Corrective action. I asked what he was doing in the gap. He said he was standing there. I asked why. He said it was part of it. I did not understand why. I have written it down. I have escalated the matter to myself.

Status. Closed. Nothing was found to correct, which I have filed under open questions.
